About Cornea Transplant

The cornea is a transparent dome-like layer on the eyeball responsible for adjusting the eye’s focusing power and seeing objects. Cornea transplant, also known as keratoplasty, is a procedure where the complete cornea or the damaged parts of the cornea are replaced with donated corneal tissues. The aim of the surgery is to improve the appearance and quality of the damaged or diseased cornea, improve vision, and reduce any pain. Pain, swelling, or distorted vision are the most common symptoms of cornea damage.

Types of Cornea Transplant

In addition to the full-thickness cornea transplant known as penetrating keratoplasty, there are two other types of cornea transplants:

Endothelial keratoplasty (EK): Doctors replace the innermost layer of the cornea, also called the endothelium.

Anterior Lamellar Keratoplasty (ALK):Doctors transplant only the outer and middle layers of the cornea.

Before the Procedure

Before the surgery, doctors will generally conduct a thorough eye examination and perform a basic health check-up to keep track of any other medications that the patient may be taking. The size of the cornea required is also measured to search for a donor cornea of the correct size. One vital step before cornea transplant surgery is to look for donors. Thanks to the increasing number of people signing up for eye donation, corneas are more readily available for transplants than other organs like the kidney or lungs.

Procedure
  • Doctors may perform the surgery under general or local anesthesia.
  • During penetrating keratoplasty, the surgeon cuts through the depth of the damaged cornea using a cutting instrument called a trephine.
  • Small disk-sized tissues are removed and replaced with the new cornea obtained from the donor.
  • Doctors stitch the tissue to ensure that it does not move or dislocate once it is placed onto the eye.
  • The stitches may be removed during a follow-up visit after some time.

Recovery after the Procedure
  • Doctors may prescribe several medications and eye drops after the surgery to reduce pain and the risk of infection.
  • Patients may need to wear an eye patch for a while after corneal transplant surgery to help the eye heal.
  • Patients should take extra precautions to avoid injury to the eye and maintain proper hygiene to ensure the eye completely heals.

Risks Associated with the Surgery

In addition to the primary risks associated with any surgery, such as the risk of infection, the primary risk of cornea transplant is corneal rejection, where the patient’s body refuses the deceased donor’s corneal tissues.
